Long-term care ombudsmen are advocates for residents of nursing homes, board and care homes, assisted living facilities and similar adult care facilities.

What is Complex Continuing Care (CCC)? In Ontario, the term “complex continuing care” (CCC) is used interchangeably with “chronic care”. Complex continuing care provides continuing, medically complex and specialized services to both young and old, sometimes over extended periods of time. CCC is provided in hospitals for people who have long-term illnesses or disabilities typically requiring skilled, technology-based care not available at home or in long-term care facilities. CCC provides patients with room, board and other basic necessities in addition to medical care. Who is eligible? Any individual who is an insured person in accordance with the requirements of the Ontario Health Insurance Plan.
